Mail-in Registration:

• Download and complete Registration and Health forms.

• Download and complete Authorization to Administer Medication Form and Alternate Pickup Form if applicable.

• Mail in all forms with full payment to address below. Please make checks payable to

• Your cancelled check is your confirmation of a reserved spot at camp. Registration is not complete and campers will be unable to attend camp until all health forms are collected.

• Overnight campers will receive detailed information prior to camp.
